Collision Repair Authorization
I authorize Iron Oak Collision to repair and operate my vehicle for inspection and testing purposes.
I understand that a mechanic’s lien may apply to secure payment for repairs. Iron Oak Collision is not responsible for personal items left in the vehicle, and I agree to remove all belongings before drop-off.
I understand rental vehicle costs are not covered. A storage fee may apply if the vehicle is not picked up within 72 hours after repair completion.
I agree that full payment is required before vehicle release and authorize my insurance company to issue payment directly to Iron Oak Collision, including any supplements. I also authorize Iron Oak Collision to endorse my name on insurance checks.
